Adult Packet

There is a ten-dollar fee for filing in Tribal Court.

On the (report of legal name change) form you fill out 1-3-4 and the bottom where to mail birth certificate.  The clerk will fill in the rest.  There is a $10.00 and a12.00 fee for filing this form in Madison.  This has to be made after the court order and provided to the clerk in cash, check or money order.
