Minor Packet

There is a ten-dollar fee for filing in Tribal Court.

On the (report of legal name change form) you have to fill out 1-3-4 question section and provide your address at the bottom of the page or where to mail birth certificate. The Clerk will file.  There is an additional fee that is required for filing this form in Madison and is contingent on the number of certificates you request of them.  This action must be done after the Judge issues the name change.
